FORGE AHEAD & HEALTH CANADA: Quality Improvement Scale-Up Toolkit


The aim of the QI Scale-up Toolkit is to assist First Nations communities across Canada to implement new activities and strategies for the prevention and management of chronic disease, such as Type 2 diabetes. The QI Scale-up Toolkit is designed to be a practical, relevant and useful resource for all community members and community-based healthcare workers. The QI Scale-up Toolkit includes a Community Participation Guide that provides an overview of the Toolkit, a set of Handbooks for each phase of the QI program, and QI Tools that communities can use to strengthen their capacity. The aim is to support changes that will improve the quality of care, allow for quicker access to and enhance the continuity of care, and create collaboration between community and clinical programs. This Toolkit is an integration of QI Tools developed by FNIHB through their Quality Resource Kit (QRK) and QI Tools developed by the FORGE AHEAD Program.

The FORGE AHEAD & Health Canada: QI Scale-Up Toolkit Development Project was made possible by funding from Health Canada, and in partnership with the First Nations Inuit Health Branch (FNIHB), with participation from its National Office and Regional Offices in Canada. The project was greatly informed by feedback from presentations made to Aboriginal Diabetes Initiative (ADI) Leads and regional members, the Chronic Disease Prevention and Management (CDPM) Working Group, FNIHB Home and Community Care Teams, the Assembly of First Nations (AFN), and Inuit Tapiriit Kanatami (ITK).

The project was guided and shaped by the QI Scale-Up Toolkit Working Group, which included members from FNIHB National Office, FORGE AHEAD Western Research Team, and Community Representatives from FORGE AHEAD Partnering First Nations Communities.
